Applications must be received by March 15 of the year for which admission is requested and must include:

  1. A completed Duke University Medical Center Allied Health application form , including a $50 non-refundable processing fee 
  2. A copy of a high school diploma or successfully completed equivalency certificate 
  3. Official transcript(s) from the most recent schools attended 
  4. Two original letters of recommendation from previous employers or course instructors 
  5. A 250-word minimum essay on your reasons or motivations for wanting to enter the program 

Following the receipt of all required information, the Admissions Committee reviews applications. Eligible applicants will be invited to Duke Eye Center for interviews in late March and early April. During the interview, candidates take a preadmission basic knowledge test. Scores from this test are one of the consideration factors for admission. It is strongly recommended that applications be submitted as early as possible. Applicants are notified of acceptance or denial no later than April 15 of the year for which applications are submitted. Classes begin the first Monday in July.
